Job summary

Presbyterian Homes & Services (PHS) in Minnesota seeks a full-time OMR Coordinator to oversee Oasis client assessments for Optage Home Care and coordinate with 30+ health professionals. The role emphasizes accurate documentation, compliance with F2F and Oasis procedures, and leadership within an interdisciplinary team to ensure high-quality, compliant care.

The position requires RN licensure, Oasis familiarity, and strong data management and communication skills.

Qualifications

• Registered nurse with current licensure in state(s) of practice.
• OASIS Certification preferred.
• Experience with Oasis completion, reimbursement coding and clinical resource utilization and/or case management.
• Ability to work flexible work hours to support business requirements.
• Competency with standard office software applications and Oasis-related tools.
• High initiative and ability to lead interdisciplinary teams in OMR process.
• Demonstrated ability to read, write, speak and understand English to communicate with all customers.

Responsibilities

• Coordinate and oversee development and completion of the Oasis client assessment process.
• Maintain schedule and assignments for Oasis assessments across client stays.
• Enter client data into OM programs and manage Oasis coding submissions.
• Audit HHCAHPS and review expedited appeals; perform concurrent Oasis reviews.
• Lead monthly QI/QM reviews and provide recommendations to the IDT and Case Managers.
• Ensure F2F compliance documentation and written plan of care for each client.
• Collaborate with IDT to communicate Oasis scoring changes and care plan updates.
