Concordia Visiting Nurses in Valencia, PA is seeking an Oasis Workflow Registered Nurse (RN) for a full-time (onsite) role reporting to the Valencia office. The RN will oversee quality measures and ensure data aligns with OASIS guidance and regulatory requirements.

Key duties include reviewing OASIS assessments, analyzing outcomes with data mining tools, improving ancillary services, auditing documentation, and training staff.

Qualifications

• Minimum of two years of home health experience.
• OASIS certification recommended; if not certified, must obtain within one year.
• Knowledge of agency policies, federal and state regulations.
• Strong interpersonal, communication, and organizational skills with evidence of leadership.
• Reliable transportation with a valid driver’s license and insurance as required by state law.

Responsibilities

• Overseeing quality measures and data to ensure submitted information aligns with OASIS guidance and regulatory requirements.
• Conduct reviews of OASIS assessments in accordance with best practice guidelines.
• Evaluate quality and process measure outcomes using advanced data mining software.
• Improve ancillary services by analyzing assessments to foster positive patient outcomes.
• Detect inconsistencies and errors in clinical documentation, reporting findings to the Quality/Education teams.
• Take part in Quarterly Quality Record Reviews.
• Work alongside the education team to provide quarterly OASIS training.
• Participate in quality and rehospitalization meetings, assessing clinician scorecards and formulating corrective action plans as needed.
• Perform audits to ensure adherence to all regulations governing home health and the appropriate use of services.
