Job summary

Pine Creek Rehab & Nursing in Salt Lake City, UT is seeking a full-time Assistant Director of Nursing and MDS Coordinator to support the Director of Nursing and oversee daily nursing operations, MDS 3.0 processes, care planning, and interdisciplinary collaboration.

The ideal candidate will have an RN license, strong leadership and documentation skills, extensive MDS knowledge, and the ability to manage multiple priorities while ensuring regulatory compliance and high-quality resident care.

Qualifications

• RN required with ADN/AS in Nursing; Bachelor's preferred.
• Extensive knowledge of MDS 3.0, RAI process, and care planning.
• Strong leadership, organizational, and documentation skills.
• Ability to supervise staff, manage schedules, and coordinate interdisciplinary teams.

Responsibilities

• Assist the Director of Nursing with daily operations and staff supervision.
• Coordinate and oversee MDS 3.0 assessments and submissions.
• Develop, review, and maintain individualized resident care plans with the team.
• Coordinate Medicare/Medicaid and insurance case management and documentation.
• Assign ICD-10-CM coding and maintain accurate records.
• Monitor regulatory compliance with federal/state rules and facility policies.
• Support quality assurance, audits, and staff education.
• Promote resident safety, dignity, and high-quality care.
