Qualifications

• RN license in the state of Florida.
• Bachelor’s degree preferred.
• Two years supervisory experience in a health care organization.
• Knowledge of hospice principles and practices.
• Working knowledge of interdisciplinary team processes and communication.

Responsibilities

• Directs patient care services for the team as part of the interdisciplinary plan of care.
• Reviews admissions, assessments, and alive discharges for eligibility and appropriateness.
• Collaborates with physicians on palliative care measures for pain and symptom management.
• Ensures complete documentation supports recertification decisions and timely updates.
• Manages team schedule and ensures 24/7 coordination of services for all patients.
• Mentors staff and oversees payroll/expenses within budget.
• Participates in on-call rotation and quality/regulatory activities.
• Maintains customer service standards and drives performance improvement.
