Job summary

Apploi is seeking a Clinical Manager to coordinate and oversee all direct care patient services provided by clinical personnel in Florida. You will assess patient needs, admit patients, assign clinicians, and lead care planning to achieve optimal outcomes.

The role requires a Nursing degree, active RN license, CPR certification, and Medicare home health experience, with at least two years as an RN and one year in management within home care or hospice.

Qualifications

• Graduate of an accredited School of Nursing.
• Current state license as a Registered Nurse.
• Proof of current CPR.
• Valid driver’s license, auto insurance and reliable transportation.
• Medicare Home Health Experience REQUIRED.
• Two years as a Registered Nurse with at least one year of management experience in a home care, hospice, or equivalent environment.

Responsibilities

• Coordinate and oversee all direct care patient services provided by clinical personnel.
• Assess and reassess patients and update care plans, interpreting patient needs according to procedures.
• Admit patients to service and assign clinicians to cases as needed.
• Assist clinicians in establishing goals and developing patient Plan of Care (POC).
• Coordinate communication between team members, attending physicians, and caregivers to ensure appropriate care and outcome planning.
• Supervise all clinical employees and manage overall direction, coordination, and evaluation.
• Participate in interviewing, hiring, and training of direct care clinicians and evaluate performance; coach staff and recommend in-service education.
