Job summary

Geisinger is seeking an RN Nurse Navigator Triage to provide nursing triage by phone and in person, evaluating acuity and directing needed medical support. The role emphasizes protocol-driven clinical advice, patient education, and timely communication to ensure quality care and continuity across services.

The position requires BSN (preferred), 5+ years nursing experience (preferred), ICU/ED experience (preferred), and proficiency with Microsoft Office and EMR tools including Word, Excel,

Qualifications

• BSN preferred.
• 5yrs nursing experience preferred.
• ICU/ED experience preferred.
• Specialty certification preferred.
• Proficient computer skills.
• Proficient with Microsoft products such as Word, Excel, PowerPoint, SharePoint, Windows, & TEAMs.

Responsibilities

• Performs a patient assessment via phone and documents patient chief complaint, symptoms, history, and contact information using established protocols.
• Initiates and coordinates emergency treatment when necessary.
• Prioritizes high priority calls addressing urgent needs and relays information to triage provider.
• Provides education to patients and families and coordinates care across departments.
