Job summary

Your Health Organization in Spartanburg, SC, seeks a Registered Nurse to join our nurse case management team. The RN will visit patients at home and in facilities, coordinate with providers, and initiate telemedicine visits as needed.

The role requires reliable transportation, travel daily, and a 8-hour shift schedule (8am–5pm) Monday through Friday. A valid RN license and 3+ years of clinical experience are preferred.

Qualifications

• Must be a licensed nurse with good standing.
• Minimum of three (3) years clinical experience preferred.
• Experience in community settings preferred.
• Proven ability to effectively communicate and collaborate with interdisciplinary care teams, patients, and caregivers.
• Strong written and verbal skills.
• Basic computer knowledge.
• Ability to manage and demonstrate effective leadership skills.
• Team player with ability to manage multiple responsibilities and sound judgment.
• Must be able to work flexible hours and travel between offices and facilities.

Responsibilities

• Facilitate receiving all medical records from the patient’s primary provider and specialists.
• Review medical records and complete consents.
• Enroll patients in Care Management if eligible.
• Initiate a Care Management Plan of Care if eligible.
• Capture diagnoses with high specificity and ensure acceptance.
• Coordinate care and communicate with providers.
• Evaluate for home health, hospice, palliative, or RPM resources.
• Complete AWVs and cognitive impairment screenings.
• Document Care Management activities and maintain up-to-date care plans.
