Your Health in Spartanburg is seeking a Registered Nurse as a Nurse Case Manager to serve patients in homes and facilities across the designated service area. Travel is required daily to visit patients, facilities, and to initiate telemedicine visits, with a salary-based schedule typical for full-time roles.

Formación

• Must be a licensed nurse with RN preferred; license in good standing.
• Minimum of three (3) years clinical experience preferred.
• Experience in community settings preferred.
• Proven ability to communicate and collaborate with interdisciplinary teams, patients, and caregivers.
• Strong written and verbal skills; basic computer knowledge.
• Willingness to travel to patients' homes, facilities, etc. and drive a vehicle.

Responsabilidades

• Facilitate receiving all medical records from the patient’s primary provider and specialists.
• Review medical records and complete consents with patients.
• Enroll patients in Care Management if eligible and initiate a Plan of Care.
• Capture all diagnoses with specificity and create gaps for acceptance.
• Complete AWV’s and cognitive impairment screenings.
• Coordinate care and reconcile medications.
• Provide patient education post-discharge and assess DME needs.
• Document Care Management activities and adapt plans as needed.
