Job summary

Legacy Health and Rehabilitation is seeking a skilled Registered Nurse (RN) to provide direct nursing care to residents in a long-term care and rehabilitation setting. The RN will support rehabilitation plans and collaborate with the clinical team to ensure high-quality care.

The role requires a valid RN license, strong geriatric care knowledge, and current BLS/CPR certification. You will work on a full-time night shift, mentoring CNAs and ensuring compassionate, patient-focused care.

Qualifications

Valid Registered Nurse (RN) license.
Strong clinical skills with geriatric and rehabilitation care.
Excellent communication and interpersonal skills.
Ability to work collaboratively in a team.
Compassionate, patient-focused approach to care.
Current BLS/CPR certification required.

Responsibilities

Deliver patient-centered nursing care in long-term care and rehabilitation.
Assess, monitor, and document residents' health conditions and progress.
Administer medications and treatments per facility protocols.
Collaborate with physicians, therapists, and healthcare professionals to develop care plans.
Educate residents and families about care plans and health conditions.
Maintain accurate documentation in line with regulations.
Foster a safe, therapeutic environment for residents.
Supervise and mentor nursing assistants to ensure quality care.
