Job summary

Legacy Health and Rehabilitation is seeking a skilled Registered Nurse (RN) to provide direct nursing care to residents in our long-term care and rehabilitation facility. You will collaborate with the clinical team to implement rehabilitation plans and ensure the highest standards of care.

You will administer medications, monitor health conditions, document progress, and educate residents and families about care plans while ensuring compliance with state and federal regulations.

Qualifications

• Valid RN license
• Experience in geriatric and rehabilitation care
• Excellent communication and interpersonal skills
• Ability to work collaboratively in a team-oriented environment
• Compassionate, patient-focused approach to care
• Current BLS/CPR certification required

Responsibilities

• Deliver patient-centered nursing care to residents in a long-term care and rehabilitation setting
• Assess, monitor, and document residents' health conditions and progress
• Administer medications and treatments as prescribed, ensuring compliance with facility protocols
• Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans
• Provide education and support to residents and their families regarding care plans and health conditions
• Maintain accurate and timely documentation in compliance with state and federal regulations
• Foster a safe, comfortable, and therapeutic environment for residents
• Supervise and mentor nursing assistants to ensure quality care delivery
