Job summary

EvergreenHealth is hiring an RN to provide direct patient care within the Neuroscience Institute on the Kirkland campus. You will coordinate care across clinic, hospital, ER and skilled facilities, triage patients, and support nurses and providers in delivering evidence-based treatment.

The role emphasizes nursing assessments, care planning and patient education, with potential involvement in the Medical Home pilot and team-based care.

Qualifications

• Graduate of a nursing program (Diploma/ADN/BSN/MSN).
• Licensed RN in Washington or an active multistate license per state requirements.
• Current Healthcare Provider BLS – American Heart Association by start date.
• 1 year of RN experience.
• Chronic care management and patient education skills.
• Ability to manage concurrent programs and time, equipment and supplies effectively.

Responsibilities

• Manages the clinic flow and ensures efficient patient throughput.
• Provides continuity of care between clinic, hospital, ER, specialists, Home Health, Hospice and LTC facilities.
• Triages patients in person and via phone.
• Assesses nursing needs and develops nursing diagnoses.
• Collaborates with providers to implement the plan of care.
• Reviews diagnostic/lab results and relays findings to providers.
• Coordinates critical results promptly with care team.
• Oversees medical assistants to ensure guideline-based care.
• May assist with Medical Home pilot activities and case management.
• Performs nursing visits and supports group visits with managers.

Wage Range: $45.19 - $82.30 per hour plus additional per diem wage

Posted wage ranges represent the entire range from minimum to maximum. For jobs with more than one level, the posted range reflects the minimum of the lowest level and the maximum of the highest level. Some positions also offer additional premiums based on shift, certifications or degrees. Join EvergreenHealth in our mission to provide patient-centered neurological care. This position will work in the state-of-the-art DeYoung Pavilion on the EvergreenHealth Kirkland campus. The Institute provides all testing, treatment and rehab in a single location. In this role, the RN will ensure that the direct nursing care of patients is conducted within established standards of care, hospital and unit policies, procedures, and guidelines. The RN will be the main resource person for the patient and will be coordinating and managing their care. Primary Duties:

1. Manages the clinic flow.
2. Provides continuity of care between the clinic, hospital, ER, specialists, Home Health, Hospice and LTC facilities.
3. Triages patients in person and via phone
4. Assesses the comprehensive nursing needs and develops a nursing diagnosis
5. Participates with the providers to develop and implement the plan of care
6. Analyzes diagnostic / lab test results and relays that information to the providers
7. Coordinates critical results in a timely manner
8. Provides oversight of medical assistants to ensure patient care is provided within established guidelines and evaluates the outcomes of care.
9. May assist with the Medical Home pilot by attending collaborative meetings, preparing month end reports, and working toward case management of patients in conjunction with the nurse case manager.
10. Performs nursing visits within scope of practice and assists with developing group visits with manager and providers.
11. Provides education and training for medical assistants in collaboration with the manager and/or RN Quality Manager.
12. Balances workloads and adjusts staffing in various pods to adequately cover providers.
13. Provides safe, family centered nursing patient care and serves as a consultant to the patients and caregivers
14. Performs other duties as assigned.

Qualifications:

License, Certification, Education or Experience:

REQUIRED for the position:

• Graduate of a nursing program (Nursing Diploma, Associate Degree in Nursing (ADN), Bachelor of Science in Nursing (BSN) degree and/or Master of Science in Nursing (MSN) degree)
• Licensed as a Registered Nurse in the State of Washington OR holding a multistate Nurse Licensure Compact (NLC) license
• Current Healthcare Provider BLS – American Heart Association by start date
• 1 year of previous RN experience
• Chronic care management and patient education skills
• Ability to manage a variety of concurrent programs and projects effectively while Utilizing time, equipment and supplies appropriately

DESIRED for the position:

• Previous lead and/or supervisory experience demonstrating effective, positive and constructive feedback skills
• Master’s in Nursing (MSN)
