Job summary

ChenMed LLC in the United States seeks a Patient Educator, Specialty (RN) to collaborate with clinicians and leaders to integrate chronic disease education into care plans for a specialized patient group. The role focuses on promoting patient engagement in care and reducing unnecessary hospital visits.

You will deliver direct specialty nursing education, coordinate with PCPs, specialists, and vendors, and develop education programs tailored to culture, language, and readiness to learn.

Qualifications

• Active RN license required; dialysis experience preferred.
• BLS certification required.
• Experience in patient education or teaching.
• Care coordination across centers essential.
• Ability to travel up to 40%.

Responsibilities

• Provides chronic disease education relative to quality care for a specific patient population.
• Coordinates care between primary care physicians, community specialists and vendor services including hospitals.
• Monitors chronic disease program performance across multiple medical centers.
• Identifies gaps and implements improvements in patient and quality outcomes.
• Assists with the development of strategies for quality improvement and management related to disease education.
• Collaborates with medical center leaders and PCPs to evaluate, develop and implement patient education programs through needs assessment analysis.
• Educates patients and/or family member(s) by understanding cultural and language needs.
• Identifies opportunities to minimize fragmentation of health care for patients.
• Encourages informed decision-making about health care options by ensuring understanding of health status.
• Maximizes care by assessing patient/family coping and adapting communication.
• Promotes healthy lifestyles to enrich overall health.
• Selects and tailors education materials to patient age, culture, language, etc.
• Documents patient care services in charts and department records.
• Implements process improvements to enhance quality results.
• Coordinates with transitional care and case management to reduce hospitalizations.
• Develops relationships with stakeholders to identify opportunities and remediation strategies.
• Performs other duties as assigned.
