ChenMed seeks a Patient Educator, Specialty (RN) to advance chronic disease education across multiple centers. You will partner with PCPs and specialists to tailor education plans, improve engagement, and reduce hospital admissions.

The role requires BSN, active RN license, 2+ years in specialty care, BLS, and teaching/training experience. Travel up to 40% nationwide is expected. English fluency essential.

Formación

• BSN degree required
• Active RN license required; Nursing Compact preferred
• Teaching/training experience required (clinical or patient education)
• 2 years’ specialty care nursing experience required
• BLS certification required
• Dialysis experience 2 years
• CNE or similar nursing certification a plus

Responsabilidades

• Provides chronic disease education relative to quality care for a specific patient population; coordinates care between PCPs, specialists, and vendors
• Monitors program performance across multiple medical centers; identifies gaps and implements improvements
• Educates patients and/or families considering cultural, language, and readiness factors
• Develops, evaluates, and implements patient education programs
• Documents patient care services in charts and department records as required by policy and law
• Coordinates care activities with transitional care and case management to reduce hospitalizations
