Qualifications

• Active Florida RN or LPN license
• Current CPR certification (BLS)
• Level 2 background screening (AHCA-compliant)
• TB screening and physical within past 6 months
• Experience in Medicare-certified home health strongly preferred
• Familiarity with EMRs (Axxess) and knowledge of PDGM & OASIS for RNs
• Excellent clinical judgment, communication skills, and time management
• Reliable transportation and commitment to punctual care

Responsibilities

• Perform skilled nursing visits: wound care, IV administration, medication management, patient assessments
• Carry out shift-based care for patients needing extended clinical support
• Monitor and document patient condition and progress
• Collaborate with physicians and interdisciplinary teams to update care plans
