Job summary

Family Pillars Homecare seeks dedicated RNs and LPNs to provide skilled nursing care in patients' homes on a per diem basis.

Roles include intermittent visits, private duty shifts, and care plan coordination with flexible scheduling and strong clinical leadership. Florida licensure, BLS, and clean AHCA background screening are required; travel to patient homes is expected.

Qualifications

• Active Florida RN or LPN license with Compact licensure accepted.
• Current CPR certification (BLS).
• Background screening (AHCA-compliant) Level 2.
• TB screening and physical within past 6 months.
• Medicare-certified home health experience preferred.
• Familiarity with EMRs (Axxess); knowledge of PDGM & OASIS for RNs.

Responsibilities

• Perform skilled nursing visits: wound care, IV administration, medication management, patient assessments.
• Carry out shift-based care for patients needing extended clinical support.
• Monitor and document patient condition and progress.
• Collaborate with physicians and interdisciplinary teams to update care plans.
• Maintain records compliant with HIPAA and CMS; OASIS documentation for RNs.
• Uphold infection control standards and agency protocols.

About the Role
