Job summary

Family Pillars Homecare seeks licensed RNs and LPNs to provide skilled nursing care in patients' homes on a flexible per diem basis in Florida. Roles include intermittent visits, private duty shifts, and care plan coordination with HIPAA compliance.

Strong communication and reliability are essential. Qualifications include active Florida RN (Compact accepted), CPR (BLS), AHCA Level 2 background check, and recent TB screen.

Qualifications

• Active Florida RN license (Compact accepted).
• Current CPR certification (BLS) present.
• Level 2 AHCA-compliant background screening.
• TB screening and physical within past 6 months.
• Experience with Medicare-certified home health preferred.
• Familiarity with EMRs and PDGM/OASIS knowledge for RNs.
• Excellent clinical judgment and communication.
• Reliable transportation and punctual care.

Responsibilities

• Perform skilled nursing visits: wound care, IV administration, medication management, patient assessments.
• Carry out shift-based care for patients needing extended clinical support.
• Monitor and document patient condition and progress.
• Collaborate with physicians and interdisciplinary teams to update care plans.
• Maintain accurate records in compliance with HIPAA and CMS guidelines (OASIS documentation for RN).
• Uphold infection control standards and agency protocols.
