Job summary

Family Pillars Homecare seeks dedicated RNs and LPNs to provide skilled nursing care in patients' homes on a per diem basis. The role includes intermittent visits, private duty shifts, and care plan coordination in Florida.

Licensed clinicians will perform wound care, IV administration, and medication management while collaborating with physicians to update plans. Expect flexible scheduling and access to training resources within a supportive team.

Qualifications

• Active Florida RN license (Compact accepted).
• Current CPR certification (BLS).
• Level 2 background screening (AHCA-compliant).
• TB screening and physical (within past 6 months).
• Experience in Medicare-certified home health strongly preferred.
• Familiarity with EMRs (e.g., Axxess) and knowledge of PDGM & OASIS for RNs.
• Excellent clinical judgment, communication skills, and time management.
• Reliable transportation and commitment to punctual, professional care.

Responsibilities

• Perform skilled nursing visits: wound care, IV administration, medication management, patient assessments.

• Monitor and document patient condition and progress.
• Collaborate with physicians and interdisciplinary teams to update care plans.
• Maintain accurate records in compliance with HIPAA and CMS guidelines (OASIS documentation required for RN).
