Job description

Responsibilities
  • Coordinate the referral process and introduction of patients into the Valve Program and/or Watchman Program.
  • Employ patient teaching to promote health and safety; evaluate progress toward outcomes; practice ethically.
  • Receive incoming referrals from physicians and hospitals and respond quickly to ensure rapid access into the system.
  • Triages referrals according to written criteria to meet patient needs on a priority level.
  • Communicate plans for acceptance into the program to referring physicians and hospitals; educate patients and families on the referral process and evaluation steps; keep them informed throughout the process.
  • Demonstrate leadership in the professional practice setting and collaborate with the inter-professional team.
  • Utilize resources to plan and provide nursing services that are safe, effective, and financially responsible; practice in an environmentally safe and healthy manner.
  • Work with providers and clinic staff to ensure smooth transition of patients during preadmission, hospitalization, discharge planning, and outpatient services.
  • Perform thorough patient assessments as required to determine suitability for procedures (clinical, psychosocial, risk scoring, frailty, mobility, KCCQ, etc.).
  • Schedule and manage testing according to protocol (e.g., Cardiac Cath, TTE, TEE) and review with the Structural Heart Coordinator or team members.
  • Act as liaison to Structural Heart Committee and Valve/WATCHMAN Coordinator Committee (data submission, monthly webinars).
  • Follow protocols for documentation and maintenance of databases related to the Structural Heart Program.
  • Participate in outreach to current and potential referring physicians; oversee internal staff education and competencies for the care of this patient population.
  • Develop and implement educational materials for patients and families; ensure consistency in information and document education provided; maintain currency with compliance training.
  • Maintain a commitment to professional growth and competency, including related certifications.
  • Assist in coordinating post-discharge clinics to evaluate patient progress and identify care issues.
  • Participate in TVT and WATCHMAN registries to ensure accurate data reporting for Quality Improvement.
  • Serve as a secondary resource for the Structural Heart Coordinator for TVT and WATCHMAN registries and identify quality issues; participate in research projects as directed.
  • Identify potential research topics related to the program and keep up to date with current literature; share information with others.
  • Provide written reports to support research projects and network with other organizations regarding valve/WATCHMAN programs.
  • Maintain and update evidence-based care protocols for valve and WATCHMAN patients; maintain a database of clinical protocols for review annually.
  • Develop and assist with outpatient care components/protocols for patients and families; navigate major aspects of care delivery for Valve/WATCHMAN Program patients.
  • Maintain open communication with NCH Healthcare Systems leadership regarding quality improvement, protocols, and finances.
Qualifications
  • Minimum of 2 years of experience in cardiac care required.
  • Licensed as a Registered Nurse (RN) in the State of Florida.
  • Basic Life Support (BLS) certification required from the American Red Cross or American Heart Association.
  • Strong communication and problem‑solving skills; ability to maintain flexibility.
  • Intermediate computer knowledge: Microsoft Word, Excel, Outlook, and Windows.
