Job summary

Kaiser Permanente Georgia is seeking an RN Navigator to guide patients and families through the care continuum, prioritizing patient education and timely access to services. Qualifications

• Minimum three years of RN clinical experience in the applicable specialty.
• Associates degree in nursing; BSN preferred.
• Georgia RN license and BLS certification required.
• Excellent communication and advocacy skills.

Responsibilities

• Assist patients and families in understanding diagnosis, treatment options, and available resources within Kaiser Permanente.
• Perform assessment, symptom management, data analysis, planning, case management, interventions, and outcome evaluation for patients with chronic conditions.
• Develop and facilitate a care plan with clinicians, patient, and family to support understanding of the plan.
• Act as a link between patient, providers, family, and care coordination teams.
• Coordinate access to services across hospital, outpatient, home, and other settings.
• Conduct care assessments and triage via phone, clinic, or coordination with the health care team.
• Schedule consults, procedures, and support services within service lines.
• Ensure pathology reports and imaging are available to support care decisions.
• Standardize patient education materials and maintain a resource library for the specialty.
