Qualifications

• 3-5 years of clinical experience required.

• 2-3 years of managed care and/or care management experience preferred.

Responsibilities

• Navigates electronic medical records and care management applications.
• Collaborates in discharge process per CMS guidelines and Population Health workflow.
• Outreach to TOC patients focusing on medication reconciliation/adherence, self-management, and follow-up with PCPs/Specialists.
