Job summary

CHRISTUS Health seeks an RN Navigator to monitor post-acute patients and ensure CMS criteria for continued Home Health services. You will coordinate with post-acute facilities, providers, and care teams to optimize patient outcomes and appropriate service levels.

Responsibilities include reviewing Plans of Care, guiding recertification decisions, and coordinating with PCPs to ensure timely, compliant care transitions. 8AM-5PM, Monday-Friday, Full Time position in Irving, TX.

Qualifications

• BSN preferred.
• 3 years clinical experience required.
• 2 years Home Health preferred.
• 3 years managed care/care management preferred.
• RN license in state of employment (or compact).

Responsibilities

• Coordinate post-acute home health reviews with CMS criteria.
• Review Plan of Care forms and Medical Necessity guidelines.
• Facilitate case conferences with post-acute agencies.
• Recommend Home Health recertification or discharge to PCPs.
• Navigate electronic medical records and care management systems.
• Support smooth transitions of care between agencies and PCPs.
• Promote collaboration with clinicians, office staff, and facilities.
