Description:The RN implements professional nursing practice centered on the patient as a whole person.A caring environment focused on healing and wellness is created.Nursing practices are based on evidence and current research.Client and departmental policies and procedures, regulatory standards, and professional guidelines are followed.Clinical decision-making incorporates all four phases of the nursing process.Critical thinking and creativity are utilized.Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines.Direct patient care activities on the unit are coordinated, ensured, and provided.This includes the provision of nursing rounds, assessments, and treatments.Facilitating admission, transfer, and discharge activities is part of the role.Patient and family-centered education is provided.Care coordination and individualized treatment planning are employed as applicable.

Responsibilities include:Performing and documenting a comprehensive assessment in accordance with the RN's scope of practice and hospital policies and procedures.Considering the patient’s culture, age, and developmental level.Identifying patient risk factors and presenting complaints.Obtaining patient health information from family or others as necessary.Collecting objective and subjective data.Verifying initial admission assessments if obtained by other members of the healthcare team.Developing a problem list, prioritizing patient needs based on input from the inter-professional team as appropriate.Designing relevant actions and interventions according to the patient’s culture and age.Determining desired outcomes in coordination with the patient and family in accordance with the medical plan of care.Fully implementing patient care therapies and procedures with empowerment and clinical autonomy within the scope of practice.Explaining procedures, treatments, and processes to patients and families.Initiating actions to alleviate patient stress and discomfort.Recognizing and responding to changes in patient condition appropriately.Conducting and documenting ongoing assessments and responses to procedures and therapies.Delegating tasks appropriately and overseeing team members in the performance of patient care activities.Monitoring the accuracy and completeness of delegated patient care provided by other team members.Identifying and addressing educational needs of patients related to prioritized problems according to comprehension levels.Facilitating patient discharge processes by executing discharge orders in a timely manner.Assuring the performance of discharge teaching and coordination of plans with Case Management and others as indicated.Providing input into patient/nurse assignments considering available staffing, unit activity, and patient acuity.Actively participating in unit-based activities in support of organizational values and priorities.Promoting a positive group dynamic and healing environment through words and actions.Assuring nursing excellence according to accepted standards, evidence-based practice, and current research.Actively participating in department and/or unit quality improvement activities and core measures.Using a holistic perspective regarding patient care.
