Job summary

Phaxis in Charlotte, NC is seeking a Registered Nurse (RN) for a primary care practice in the SouthPark area. The role offers a consistent Monday-Friday outpatient schedule, hands-on clinical care, patient education, care coordination, and nurse triage.

You will assess patients, document in the EHR, administer medications and IVs, educate patients and families on meds and chronic disease management, and support staff to maintain smooth patient flow during busy periods.

Qualifications

• Active RN license in North Carolina.
• Outpatient primary care experience preferred.
• Strong patient education and care coordination skills.

Responsibilities

• Assess patients, obtain histories and vital signs, and document findings in the EHR.
• Carry out provider-ordered treatments and administer medications.
• Start IVs, administer IV fluids, and monitor patient responses.
• Educate patients and families on medications, inhaler use, and chronic condition management.
• Complete refills, prior authorizations, forms, and electronic orders.
• Monitor quality data related to acute and chronic care.
• Serve as a clinical resource for walk-in emergencies and participate in emergency response procedures.
• Support Medical Assistants and maintain efficient patient flow during busy periods.
• Nurse triage: evaluate calls, assess symptoms, and route to appropriate care.
