Responsibilities

  • Discharge planning and patient education, especially renal/fluid management
  • Coordinate care with interdisciplinary teams across units
  • Post-operative monitoring and wound care as needed

Registered Nurse - Medical-Surgical/Telemetry
Facility Overview
  • 637 licensed beds
  • Level 1 Trauma Center
  • Teaching Hospital

Unit Details
  • Number of beds: 35
  • Typical census: 35
  • Minimum experience: 2 years in Medical-Surgical/Telemetry nursing
  • First-time travel nurses are welcome
Patient Population and Common Diagnoses
  • Orthopedic and trauma cases including fractures (femur, tibia-fibula, humerus, spine, ribs, etc.), head trauma with hemorrhage, osteomyelitis
  • Patients with acute and chronic illnesses
  • Post-operative surgical patients
  • Common co-morbidities in the med-surg population: diabetes, hypertension, infections, congestive heart failure, electrolyte imbalances
Staffing Ratios
  • Registered Nurse: 1:4-6
  • Licensed Practical Nurse: 1:4-6
  • Certified Nursing Assistants: 2-3 per shift
Licensure and Certifications
  • BLS certification required
  • Basic Cardiac Rhythm (BCR) certification required; travelers must pass an EKG competency exam for compliance
  • ACLS certification preferred
  • National certification in Medical-Surgical or Nephrology nursing preferred
Required Clinical Skills
  • Care of tracheostomies, chest tube management, peripheral IV insertion, use of Glucommander, management of insulin and heparin drips, titration of drips affecting heart rate (not blood pressure)
  • Strong clinical assessment and med-surg nursing skills appropriate for a Level 1 Trauma Center
  • IV insertion and maintenance, central line care
  • Wound care and post-operative patient monitoring
  • Cardiac monitoring and telemetry interpretation
  • Coordination of care with interdisciplinary teams
  • Discharge planning and patient education, particularly related to renal and fluid management
Departmental Support
  • CNA responsibilities include vital signs, glucose monitoring, bathing, feeding, ambulation, patient turning, hourly rounding, and patient transport (transport team available); also responsible for obtaining blood from the blood bank
  • Unit secretary available from 7 a.m. to 11 p.m.; house-wide call secretary from 11 p.m. to 7 a.m.
  • Charge nurse may have patient assignments
  • Phlebotomy primarily performed by laboratory staff; nurses draw blood from central venous lines
  • No dedicated IV/VAT team, but a PICC team is available
  • Resource nurses include discharge nurses and a float team covering all units
  • Respiratory therapy available 24/7
  • Hospitalists available 24/7
  • Intensivists available 24/7
  • Central Telemetry Unit provides telemetry monitoring and technical support

  • Unit-specific equipment includes VS3 vital sign monitors, Nova Glucose Stat Strips, cardiac telemetry monitors
  • Portable vital signs monitors (Dynamapp)
  • Bladder scanners, IV pumps, portable oxygen systems
  • External fixators, chest tubes, Jackson-Pratt drains, Hemovac drains
  • Glucommander system
  • PCA pain management devices
  • Suction machines
  • Use of admission and discharge protocols to facilitate rapid patient turnover
Floating Policy
  • Floating occurs across all adult medical-surgical and telemetry units within the facility.
Orientation Process
  • One-day orientation including educational modules
  • Two days of departmental orientation
Scheduling Details
  • Weekend rotation: 5-6 weekend shifts every 12 shifts on a 6-week schedule
  • No on-call responsibilities
  • Holiday work expectation: one shift during major holidays (Thanksgiving or the day after, December 24 or 25, December 31 or January 1)
  • Return-to-office requests require manager approval
  • Shift length: 12-hour shifts, either 7 a.m. to 7 p.m. or 7 p.m. to 7 a.m.
  • Schedule cycles every 6 weeks
