Qualifications

• Must possess a current, valid RN license in state of practice.
• BLS certification is required.
• Experience in acute care nursing is desired.
• Bachelor's degree preferred; professional certification preferred may be required or encouraged.

Responsibilities

• Assess patient physical, psychological, social and discharge planning needs.
• Formulate and implement a plan of care with the healthcare team.
• Provide direct patient care and coordinate activities of the care team.
• Document care plans, assessments and patient responses in the medical record.
• Promote evidence-based practice and contribute to quality improvement initiatives.
