Qualifications

• Direct patient care, evaluates outcomes, and adjusts nursing care as needed.
• Monitors patient condition and reports changes to the team.
• Reports abnormal data to physician or supervisor as appropriate.
• Performs technical nursing procedures competently; seeks help when needed.
• Develops and documents individualized care plans for patients.
• Assesses educational needs and documents patient teaching.

• Monitors medical condition of patients and reports changes to appropriate personnel.
• Reports abnormal diagnostic data to physician/shift coordinator/supervisor as appropriate.
• Demonstrates ability to perform technical nursing procedures/skills of patient care competently, seeking assistance when appropriate.
• Implements patient histories, assesses patients' conditions, and develops individualized care plans for assigned patients. Documents on care plans as patient care evolves, including resolution of problems.
• Assesses patient educational needs and barriers to learning. Implements multidisciplinary patient education care plan and documents patient teaching.
