• Assessment: admission and ongoing assessment of patient’s condition and needs.
• Planning: design, direct, evaluate and document care plans and discharge planning with the care team.
• Implementation: prepare patients for treatments and procedures; report changes; adhere to safety protocols.
• Evaluation: reassess patients after interventions and adjust care as needed.
• Report Skills: provide comprehensive bedside reports for oncoming shifts and coordinate care.
• Environment & Patient Safety: maintain safe environment and address safety hazards promptly.
