Job summary

Hanover Terrace is seeking an MDS Nurse to join our caring team in New Hampshire. You will lead MDS assessments, coordinate interdisciplinary care plans, and ensure compliant documentation across long-term, sub-acute, and memory care units.

As a valued member of a family-owned facility, you will collaborate with physicians, therapists, and nursing staff to deliver exceptional resident-centered care. A NH RN license and HIPAA compliance are required.

Qualifications

• Licensed Registered Nurse in New Hampshire required.
• Experience with MDS assessments and care planning preferred.
• Familiarity with HIPAA regulations and patient privacy.

Responsibilities

• Facilitates and documents weekly skilled Medicare meetings.
• Transmits MDS to CMS and resolves validation issues.
• Completes PPS MDS assessments within regulatory timeframes.
• Maintains PPS schedule and accurate resident records.
• Collaborates with care teams to ensure comprehensive resident care.
• Performs other assigned duties as needed.

Your strong clinical skills and commitment to resident-centered care will help us meet the evolving needs of those we serve.If you are ready to join a compassionate team that values your contributions and growth, we want to hear from you!RESPONSIBILITIESIncluding, but not limited to:Facilitates weekly skilled Medicare meetings and provides clear communication regarding skilled coverage criteria.Transmits MDS to CMS and follows up to validate any errors or messages revealed on final validation reports.Completes all resident Interviews and PPS MDS assessments within regulatory time frames including CAAs.Drives the development of the Interdisciplinary Care Plan and updates subsequent MDS Assessment.Participates and reviews invoices as indicated for skilled patients under consolidated billing.Maintains a current PPS schedule while setting all PPS MDS ARDs within time frames in the MDS software program.Maintain accurate resident records and documentation in compliance with state and federal regulations, while ensuring compliance with HIPAA regulations.Participate in case management activities to ensure continuity of care.Provide support in specialized areas such as PT, OT, SUD, Pain Management, etc..Collaborate with healthcare team members to deliver comprehensive resident care in our facilities.All other duties as assigned.SKILLSRegistered Nurse in the StateEMR: PCC (Point Click Care)Proficiency in memory care techniques and protocols.Knowledge of MDS (Minimum Data Set) assessments for long-term care facilities.Familiarity with operations in Skilled Nursing, Sub-Acute, and Dementia Units.Strong background in case management practices.Ability to work effectively in a medical office environment.Knowledge of HIPPA, and state and federal regulations.Excellent interpersonal skills for effective communication with patients, families, and healthcare teams.Strong organizational skills to manage multiple tasks efficiently.We are proud to be an equal opportunity workplace and an affirmative action employer.
