Job summary

Aspire Physical Recovery Center at Hoover is seeking an experienced RN MDS Coordinator to join our clinical leadership team. This role coordinates the resident assessment process and ensures accurate, timely completion of the MDS and related documentation.

You will collaborate with nursing, therapy, dietary, social services, and physicians to coordinate assessments, support individualized care planning, and promote compliance with facility standards.

Qualifications

• Current RN license in good standing.
• Strong clinical assessment and critical-thinking skills.
• Excellent organization and attention to detail.
• Ability to manage multiple deadlines and priorities.
• Proficiency with electronic clinical documentation systems.
• Ability to work effectively with an interdisciplinary healthcare team.

Responsibilities

• Coordinate and complete MDS assessments within required timeframes.
• Review clinical documentation for accuracy of resident conditions and services.
• Schedule assessments and coordinate interdisciplinary participation.
• Collaborate with nursing, therapy, dietary, social services, physicians, and care team.
• Develop and review individualized resident care plans.
• Monitor documentation for completeness, consistency, and accuracy.
• Identify documentation concerns and arrange clarification or follow-up.
• Support accurate reimbursement through clinical assessment and documentation.
• Monitor resident changes requiring additional assessment or care-plan review.
• Participate in interdisciplinary meetings and care conferences.
• Assist with audits, quality improvement initiatives, and survey readiness.
• Serve as a resource to nursing staff on MDS requirements.
• Maintain confidentiality of resident and clinical information.
