Glendale Memorial Hospital and Health is seeking an experienced MDS Coordinator - RN to lead the MDS completion for Subacute residents from admission to discharge, ensuring accurate documentation and timely transmission.
The role collaborates with physicians, long-term care partners, and regulatory agencies, identifies patient problems, updates care plans, coordinates weekly interdisciplinary meetings, and maintains strict confidentiality while supporting the hospital's mission.
As our MDS Coordinator - RN is responsible for program development for specialty Subacute care unit program through education to the community including healthcare providers and managed care organizations. This position acts as the MDS RN Coordinator and has the responsibility for the completion and transmission of MDS for Subacute residents. The MDS Coordinator integrates the program OBRA constituencies thereby improving the continuum of patient care. This position requires the full understanding and active participation in fulfilling the Mission of Community Hospital of San Bernardino.It is expected that the employee will demonstrate behavior consistent with the Core Values.The employee shall support strategic plan and the goals and direction of the quality improvement, process, and activities.
