Job summary

Corona Regional Medical Center is seeking an MDS Coordinator (RN) to coordinate the interdisciplinary team for timely completion and accurate transmission of the Minimum Data Set and related assessments and care plans. The role reports to the Director of Subacute Nursing and ensures schedules are followed to prevent reimbursement issues.

Ideal candidates hold an active California RN license, have SNF experience, and possess CPR/BLS certification.

Qualifications

• Current California RN license required.
• Two years recent SNF experience preferred.
• Minimum six months experience as an MDS Coordinator preferred.
• Current BLS (CPR) certificate required.

Responsibilities

• Coordinate interdisciplinary team to complete, verify and transmit MDS and related assessments and care plans on schedule.
• Ensure MDS transmissions occur according to set schedules to prevent reimbursement issues.
