Job summary

Deer Lodge Nursing and Rehabilitation in Deer Lodge, MT is seeking an MDS Coordinator to join our team. This in-person role focuses on coordinating resident assessments and care planning to deliver high-quality resident care and compliance with regulations.

The MDS Coordinator will work with nursing, PT, nutrition, and other staff to develop individualized care plans while ensuring Medicare and federal/state guidelines are met. RN license is required and background checks are required.

Qualifications

• RN license required.
• Experience working in a Skilled Nursing Facility preferred.
• Must be able to pass a background check.

Responsibilities

• Assess residents to create individualized care plans and coordinate with nursing, PT, and nutrition to ensure guidelines are followed.
• Ensure assessments comply with Federal and State regulations and Medicare requirements.
