Job summary

Gallatin County is seeking an MDS Coordinator for the Gallatin Rest Home in Montana. The role focuses on coordinating resident assessments, developing individualized care plans, and ensuring CMS PDPM compliance.

The candidate will maintain accuracy in documentation, support interdisciplinary teams, and participate in care conferences. A Montana RN license with a BSN is required, along with 3+ years of related experience.

Qualifications

• Bachelor's degree in nursing (NLN-accredited).
• Current Montana RN licensure.
• Minimum 3 years of related clinical experience or equivalent.
• RAC-CT certification preferred.

Responsibilities

• Coordinate completion of MDS for each resident assessment per CMS PDPM requirements.
• Develop and update resident care area narrative assessments and care plans with goals and interventions.
• Participate in Medicare Utilization Review meetings and coordinate with therapy staff for plan modifications.
• Organize care conferences, review EHR data, and document conference notes.
• Assist with staff education on MDS accuracy, documentation standards, and care planning best practices.
