Trinity Manor in Kansas is seeking an RN MDS Coordinator for a part-time role. The position coordinates accurate MDS assessments and contributes to comprehensive care planning within a multi-disciplinary team.

The role requires an active Kansas RN license, MDS and care-planning experience, and knowledge of RAI guidelines, PDPM, and Medicare requirements. Strong organizational and communication skills are essential.

Qualifications

• Current Kansas RN license required.
• MDS and care-planning experience required.
• Knowledge of RAI guidelines, PDPM, and Medicare requirements expected.

Responsibilities

• Complete and coordinate MDS assessments accurately and within required timelines.
• Develop, review, and update individualized resident care plans.
• Coordinate care-plan meetings with residents, families, and the interdisciplinary team.
• Monitor documentation to support resident needs and reimbursement.
• Review clinical records for accuracy and regulatory compliance.
• Track Medicare and managed-care residents, including assessment schedules and required documentation.
• Participate in utilization review, quality improvement, and compliance activities.
• Educate nursing staff regarding documentation requirements and changes in resident conditions.
• Communicate with physicians, nursing leadership, therapy, residents, and families.
• Assist with audits, surveys, and other regulatory reviews.

Now Hiring: RN MDS Coordinator

Schedule: Part Time

About the Role:

The MDS Nurse coordinates and completes accurate, timely resident assessments in accordance with federal and state regulations. This position works closely with nursing, therapy, social services, dietary, and other members of the interdisciplinary team to ensure each resident’s needs, services, and clinical conditions are accurately reflected in the MDS and care plan.

Requirements

• Current Kansas RN license in good standing
• Long-term care or skilled nursing experience
• MDS and care-planning experience
• Knowledge of RAI guidelines, PDPM, and Medicare requirements
• Strong organizational, communication, and time-management skills
• Ability to meet deadlines and work effectively with an interdisciplinary team
