Job summary

Starr County Nursing and Transitional Care in Rio Grande City, TX seeks an experienced MDS Coordinator (RN) to ensure accurate assessments, timely documentation, and individualized care planning aligned with CMS requirements. You will collaborate with rehab and interdisciplinary teams to optimize reimbursement and resident outcomes.

The role requires an active TX RN license and at least two years in skilled nursing, with strong attention to detail, organization, and communication.

Qualifications

• Active RN license in Texas in good standing.
• Minimum two years of skilled nursing experience.
• Knowledge of MDS 3.0, RAI guidelines, PPS assessments, ARDs, and care plans.

Responsibilities

• Ensure accurate, timely MDS assessments (Admission, PPS, Quarterly, Annual, Significant Change).
• Complete Medicare/Medicaid case-mix documentation for reimbursement.
• Collaborate with Director of Rehab and interdisciplinary team for care planning.
• Track eligibility and benefit utilization for Managed Care residents.
• Audit MDS coding and supporting documentation for CMS compliance.
• Provide training on MDS/RAI processes to staff.
• Support ADRs, audits, and documentation requests as needed.
