Wellsential Health is seeking an MDS Nurse (RN) to support Starr County Nursing and Transitional Care in Rio Grande City, TX. The MDS Coordinator ensures accurate assessments, documentation, care planning, and regulatory compliance to optimize reimbursement and resident care.

The role requires RN licensure in Texas, at least two years in skilled nursing, and strong collaboration with the interdisciplinary team.

Qualifications

• Active RN license in Texas in good standing.
• Minimum two years of skilled nursing experience.
• Knowledge of MDS/RAI, Medicare/Medicaid case-mix, care planning, and clinical reimbursement.

Responsibilities

• Ensure accurate MDS assessments per CMS guidelines.
• Prepare Medicare/Medicaid case-mix documentation for reimbursement.
• Collaborate with Director of Rehab and interdisciplinary team on ARDs and care planning.
• Audit MDS coding and submission for accuracy and compliance.
• Train staff on the RAI process and regulatory updates.
