Job summary

Continental Care and Rehabilitation in Montana is seeking an experienced MDS Nurse (RN/LPN) to lead the MDS/RAI process, ensure accurate assessments, and support regulatory compliance in a long-term care setting.

You will coordinate care plans, collaborate with physicians and care teams, and educate staff on documentation and PDPM requirements to optimize resident care and reimbursement.

Qualifications

  • Active Montana RN or LPN license.
  • Minimum of three (3) years of nursing experience.
  • At least one (1) year of skilled nursing experience.
  • Strong clinical assessment and documentation skills.
  • Excellent communication, organizational, and time management abilities.
  • Intermediate computer proficiency, including electronic medical records and Microsoft Office.

Responsibilities

  • Coordinate the facility's Resident Assessment Instrument (RAI) process in compliance with state and federal regulations.
  • Complete accurate and timely MDS assessments and supporting clinical documentation.
  • Develop, implement, and evaluate individualized resident care plans.
  • Schedule and facilitate interdisciplinary care plan meetings.
  • Audit medical records to ensure documentation supports MDS coding and reimbursement.
  • Collaborate with physicians, health plans, and interdisciplinary team members to support insurance approvals and coverage requirements.
  • Serve as the facility resource for Medicare reimbursement, PDPM, quality reporting, and regulatory compliance.
  • Schedule ARDs for OBRA and Medicare assessments.
  • Submit and validate MDS transmissions through the QIES ASAP system.
  • Correct transmission errors promptly to maintain compliance.
  • Participate in Triple Check, utilization review, QAPI, and quality improvement activities.
  • Assist with admissions, discharges, and resident care as needed.
  • Provide education and guidance to clinical staff regarding documentation and regulatory requirements.

This position plays a vital role in ensuring accurate resident assessments, regulatory compliance, quality outcomes, and comprehensive care planning. Position Summary

The MDS/CRC Coordinator is responsible for coordinating and managing the Resident Assessment Instrument (RAI) process in accordance with federal and state regulations. This role oversees the accurate completion of Minimum Data Set (MDS) assessments, develops and evaluates resident care plans, supports reimbursement processes, and serves as the facility's clinical resource for Medicare, Medicaid, and quality reporting requirements.

Qualifications
Required:
  • Current, active, unencumbered Nurse (RN/LPN) license in the state of Montana

  • Minimum of three (3) years of nursing experience desired

  • At least one (1) year of skilled nursing experience desired

  • Strong clinical assessment and documentation skills

  • Excellent communication, organizational, and time management abilities

  • Intermediate computer proficiency, including electronic medical records and Microsoft Office

Preferred:
  • Previous MDS Coordinator experience

  • Resident Assessment Coordinator (RAC) Certification

  • RAC training (or willingness to complete within six months of hire)

Key Responsibilities
