Zusammenfassung

Apploi is seeking an MDS Coordinator in New Jersey to join our dedicated post-acute care team. In this role you will coordinate MDS assessments and care plans, ensuring Medicare/Medicaid schedules are met and data integrity is maintained.

You will lead interdisciplinary collaboration, monitor eligibility and RUGs, and support staff through education on MDS terminology and reimbursement regulations. A RN license in NJ and LTC experience are required.

Qualifikationen

• Licensed RN with New Jersey licensure is required.
• Minimum 2+ years of LTC nursing experience.
• Experience with MDS 3.0, RAI processes, and Medicare/Medicaid reimbursement.
• BSN preferred.

Aufgaben

• Coordinate MDS assessments and care plans within Medicare/Medicaid schedules.
• Track Medicare eligibility and RUGs; perform concurrent reviews for accuracy.
• Oversee electronic transmission of MDS data to state and federal agencies.
• Lead daily admission and care planning meetings; communicate acuity changes to leadership.
• Audit medical records to ensure accurate documentation and coding.
• Provide staff training on MDS terminology, documentation, and reimbursement rules.
