Job summary

Vantage Healthcare has an immediate opening for an RN – MDS Coordinator at Chestnut Hill of East Longmeadow. You will perform federally mandated assessments and compile MDS reports to support resident care planning. You’ll work with an interdisciplinary team to ensure compliant, high-quality care in a long-term care setting.

Qualifications

• RN with a minimum of 1 year MDS experience in a LTC facility.
• Bachelor’s degree in nursing from an accredited college is required.
• Knowledge of MDS, RAI, and care planning processes.
• Familiarity with Medicare/Medicaid regulations and compliance.

Responsibilities

• Conduct federally mandated assessments of residents using RAI.
• Collect data and compile it into a comprehensive MDS report.
• Develop and monitor individualized care plans with interdisciplinary team.
• Coordinate with insurance and management to ensure compliance and documentation.
