Job summary

Complete Care at Green Knoll in Bridgewater, MA is seeking an RN MDS Coordinator to lead a compassionate team, ensure accurate MDS assessments, and uphold regulatory compliance to deliver high-quality resident care.

You will collaborate with interdisciplinary teams, support billing processes, and provide staff education. LPNs with MDS experience may be considered for the role.

Qualifications

• Associates or Bachelor's degree in Nursing required.
• Active, unencumbered New Jersey RN license.
• 2 years proven MDS Coordinator experience in long-term care preferred.
• Experience coordinating MDS assessments and care plans.

Responsibilities

• Lead and coordinate MDS assessments for residents per federal/state regulations.
• Collaborate with Interdisciplinary teams to gather data for resident assessments.
• Ensure accuracy of MDS to support care planning and regulatory compliance.
• Educate staff on MDS processes and documentation requirements.
• Work with nursing/administrative staff on billing and reimbursements.

What You'll Do:

Registered Nurse-RN-MDS Coordinator Qualifications and Responsibilities:

• Associates or Bachelor's degree in Nursing
• Active, unencumbered New Jersey RN license
• Previous long-term care experience is preferred
• 2 years proven experience as an MDS Coordinator in a long-term care setting.
• Conduct and coordinate the Minimum Data Set (MDS) assessments for residents in accordance with federal and state regulations.
• Collaborate with interdisciplinary teams to gather information for comprehensive resident assessments.
• Ensure accuracy and completeness of MDS assessments to support resident care planning and regulatory compliance.
• Stay updated on changes in regulations related to MDS assessments and implement necessary adjustments.
• Actively participate in care planning meetings and contribute to the development of individualized resident care plans.
• Monitor and track resident progress, updating assessments as needed to reflect changes in health status.
• Work closely with nursing and administrative staff to facilitate accurate billing and reimbursement processes.
• Provide education and training to staff on MDS assessment processes and documentation requirements.
• LPN with MDS experience MAY be considered
