Job summary

Complete Care in New Jersey is seeking a Registered Nurse-RN-MDS Coordinator to lead accurate MDS assessments and personalized care plans. You will collaborate with an interdisciplinary team to ensure quality resident care, regulatory compliance, and appropriate reimbursement.

The ideal candidate holds an Associate's or Bachelor's in Nursing, an active NJ RN license, and prior long-term care or MDS experience.

Qualifications

• Associate's or Bachelor's Degree in Nursing.
• Current, unrestricted New Jersey RN license.
• Long-term care experience preferred.
• RN-MDS Coordinator or MDS assessment experience in skilled nursing preferred.
• Strong clinical assessment, communication, and organizational skills.
• Knowledge of federal and state MDS regulations.
• Ability to work collaboratively with interdisciplinary teams.

Responsibilities

• Coordinate and complete accurate MDS assessments per guidelines.
• Develop individualized resident care plans with interdisciplinary teams.
• Ensure documentation supports quality care, compliance, and reimbursement.
• Monitor MDS schedules and update documentation as conditions change.
• Participate in care plan meetings and contribute clinical expertise.
• Educate and support clinical staff on MDS processes and updates.
• Work with leadership to maintain compliance and drive quality improvement.
• Help Complete Care deliver resident-centered care.

Registered Nurse-RN-MDS Coordinator Qualifications

• Associate's or Bachelor's Degree in Nursing

• Current, unrestricted New Jersey Registered Nurse (RN) license

• Previous long-term care experience preferred

• Experience as a Registered Nurse-RN-MDS Coordinator or MDS assessment experience in a skilled nursing setting preferred

Registered Nurse-RN-MDS Coordinator Responsibilities

As a Registered Nurse-RN-MDS Coordinator, you will:

• Coordinate and complete accurate MDS assessments in accordance with federal and state guidelines.

• Partner with nursing, therapy, social services, and other departments to develop individualized resident care plans.

• Ensure documentation supports quality resident care, regulatory compliance, and reimbursement accuracy.

• Monitor assessment schedules and update MDS documentation as residents' conditions change.

• Participate in care plan meetings and contribute clinical expertise to improve resident outcomes.

• Educate and support clinical staff on MDS processes, documentation standards, and regulatory updates.

• Work closely with leadership to maintain compliance and promote continuous quality improvement.

• Help ensure Complete Care continues to provide the highest level of resident-centered care.
