Job summary

Essex Meadows is seeking an experienced MDS Coordinator (RN) to oversee twenty-four-hour nursing care and ensure a person-centered system of care for residents in Connecticut.

You will coordinate care planning, optimize PDPM reimbursement, and maintain regulatory compliance while guiding the nursing staff.

Qualifications

• Current Connecticut RN license and CPR/BLS.
• Experience with MDS software and CMS regulations.
• Ability to lead interdisciplinary care planning.
• Supervisory experience preferred.

Responsibilities

• Coordinate and lead interdisciplinary care planning with Nursing, Therapy, Social Services, Dietary, and Activities.
• Lead IDT meetings including Daily PDPM and Care Plan Conferences.
• Submit accurate, timely MDS assessments per CMS requirements.
• Monitor PDPM components and ensure regulatory compliance.
