Job summary

Essex Meadows Inc in Essex, CT seeks an experienced MDS Coordinator (RN) to oversee 24-hour nursing care through skilled MDS assessments and care planning. The role ensures quality of life for residents, maintains regulatory compliance, and optimizes PDPM reimbursements.

The position requires CT RN licensure, BLS, and experience with MDS software. Competitive benefits and a flexible, full-time schedule are offered.

Qualifications

Must have current and valid Connecticut RN license and BLS certification.
Minimum 6 months nursing experience in long-term care preferred.
In-depth experience with MDS software and transmittal process.
Knowledge of regulations impacting the MDS and supervisory experience preferred.

Responsibilities

Coordinate interdisciplinary care planning with Nursing, Therapy, Social Services, Dietary, and Activities.
Lead IDT meetings including Daily PDPM and Care Plan Conferences.
Submit accurate, timely MDS assessments per CMS guidance.
Select ARDs and diagnosis codes for PDPM classification and reimbursement.
Monitor PDPM components and ensure CMS compliance.
Perform other duties as assigned.
