Qualifications

• Familiar with electronic records and basic software.
• Able to follow directions and work independently.

Responsibilities

• Coordinate with clinical staff and therapy for documentation and care plans.
• Research medical information for optimal reimbursement.
• Ensure timely certifications and sign-offs on orders.
• Meet Managed Care organization requirements.
• Lead daily meetings with therapy and staff on resident progress.
• Monitor nursing documentation and provide ongoing education.
• Support resident assessment processes per MDS requirements.
• Ensure ADL documentation and provide training.
• Audit MDS documents for accuracy and dates.
• Submit MDS forms promptly and schedule care-planning meetings.
• Assist on floor and serve as on-call/manager on duty.
