Job Summary
We are looking for a full‑time MDS Coordinator (RN) to join our team. Training opportunity for interested candidates. RN license required. RNAC certification provided after six months of employment.
Responsibilities
- Coordinate with clinical staff and therapy for proper documentation, care programs, and optimal reimbursement throughout the stay of a skilled resident.
- Research all pertinent medical information and diagnoses for optimal reimbursement.
- Ensure timely certifications/re‑certifications and signing of all orders.
- Meet all Managed Care organization requirements.
- Conduct daily meetings with therapy and line staff on progress of skilled residents.
- Monitor nursing documentation and provide ongoing education as needed.
- Participate in the development and implementation of the resident assessment process in accordance with Minimum Data Set requirements.
- Ensure proper ADL documentation and provide education/training.
- Audit and analyze MDS documents for accuracy and completion dates.
- Submit all MDS forms promptly.
- Oversee and schedule resident care‑planning meetings.
- Assist on floor and serve as on‑call/manager on duty as needed.
- Attend all required in‑service training and education sessions as scheduled.
- Perform additional duties as assigned.
Physical Demands & Work Environment
- Active position requiring frequent standing, walking, bending, kneeling, stooping, and climbing.
- Must lift or carry up to 50 lbs. and push up to 250 lbs. independently.
- Use proper body mechanics to assist residents in daily living.
- May involve long hours, nights, and weekend work.
Travel
No travel is expected. Occasional travel may be required for training sessions, continuing education, emergency situations, and other company functions.
Education and Experience
- Four‑year college or university degree desired.
- Current state licensed Registered Nurse.
- Understanding of diagnosis coding.
- Working knowledge of prescription drugs, therapy practices, and bio‑psycho‑social sciences.
- Maintain good standing with all Nurse Practice Act standards.
- Two years’ experience in a similar healthcare position preferred.
Knowledge, Skills, and Abilities
- Fluency in reading, writing, speaking, and understanding English.
- Ability to meet or exceed company attendance and punctuality standards.
- Comfort with electronic records, software, and office equipment.
- Ability to follow directions and work independently.
- Capability to work with minimal supervision.
Licenses & Certifications
- Registered Nurse license mandatory.
