Job summary

Signature HealthCARE seeks an MDS Coordinator to lead the RAI/MDS process in our long-term care and post-acute communities. You’ll complete and manage accurate assessments, coordinate interdisciplinary care planning, and support regulatory compliance and reimbursement.

You will educate staff on MDS and documentation standards, and contribute to quality improvement initiatives that enhance dignity, outcomes, and quality of life for our residents.
