Job summary

Rehabilitation Center of Independence West Campus is seeking a MDS Coordinator to join our skilled nursing team. You will develop, monitor, and modify each resident’s care plan to meet individual needs and goals, and you will perform resident assessments and assist in the discharge process.

The ideal candidate has an unencumbered Iowa RN license, a nursing degree, CPR certification, and prior MDS Coordinator experience. CNAs with RN experience are encouraged to apply.

Qualifications

• RN license (Iowa) required.
• Graduate of an accredited nursing program.
• CPR Certification required.
• Prior experience as an MDS Coordinator preferred.
• One year RN experience in a Skilled Nursing Facility preferred.
• One year CNA experience preferred.

Responsibilities

• Develop, monitor, audit, and modify each resident’s care plan.
• Perform resident assessments and assist in discharge planning.
