Job summary

American Medical Associates is seeking an MDS Nurse Coordinator for our long-term care facility in Warren, Ohio. The role focuses on completing assessments, MDS, RAPs, and care plans for residents, and ensuring timely documentation and regulatory compliance.

You will lead multidisciplinary teams, monitor MDS completion within deadlines, and conduct care plan conferences with emphasis on accuracy and quality of resident outcomes.

Qualifications

• Must hold an Ohio RN license.
• Excellent leadership skills.
• MDS experience in long-term care.
• Clear understanding of MDS.
• Must know MDS 3.0.

• Initiates care plans and supporting activities that will result in best possible outcome for assigned residents.
• Generates and distributes monthly care plan calendar for the following month.
• Conducts care plan conferences for assigned residents.

Salary Range: $90K to $100K depending on experience
