Job summary

Mont Marie Rehabilitation and Healthcare Center seeks an experienced MDS Coordinator to ensure timely, accurate, and compliant completion of all MDS assessments, supporting resident care planning, reimbursement, and regulatory compliance. You will supervise data entry, validate results, and collaborate with the DON and care teams in a 84-bed sub-acute and long-term care facility.

The ideal candidate is a graduate of an accredited School of Nursing with an active RN license, at least 3 years in

Qualifications

• RN license (active) and nursing degree.
• Minimum of 3 years of clinical experience in a long-term care setting.
• Prior MDS/RAI experience required.
• Strong clinical assessment skills with attention to detail and accuracy.
• Working knowledge of Medicare and Medicaid regulations and reimbursement systems.

Responsibilities

• Ensure timely and accurate completion of all MDS assessments in accordance with regulations and deadlines.
• Verify compliance with federal, state, and facility regulations for MDS documentation and submission.
• Supervise MDS data entry, validation, and transmission processes.
• Identify, investigate, and resolve data discrepancies and validation issues.
• Prepare and present MDS-related reports to the Director of Nursing and leadership team.
• Collaborate with interdisciplinary teams to ensure accurate representation of resident needs.
