Job summary

Shady Lawn is seeking a qualified MDS Coordinator - RN to lead MDS development and care planning. You will coordinate assessments, ensure timely MDS transmission, and maintain quality assurance for the process.

The role requires an active RN license in the state, experience in rehabilitative or long-term care nursing, and strong English communication plus computer/documentation skills. Strong coordiantion with care teams is expected.

Qualifications

• Must possess a current, unencumbered, active license to practice as a RN in the state.
• Experience in rehabilitative and restorative nursing or as a staff nurse in long-term care.
• Must be able to read, write, speak and understand English; computer and documentation software skills required.

Responsibilities

• Coordinate the development and completion of the MDS according to current rules and guidelines.
• Manage the MDS process and care plans; ensure timely transmission.
• Develop and maintain an ongoing quality assurance program for assessments and care plans.
• Lead interdisciplinary team meetings and ensure plan of care reflects changes and updates.
